PHYSICAL FORM:  All student-athletes at LMS need an updated physical form to participate in interscholastic sports.  These physical forms are valid for one year from the date that the physician performed it. 

INSURANCE WAIVER:  All student-athletes at LMS need an updated insurance waiver on file with EACH sport season.  Please fill these out immediately prior to the beginning of the season and fill a new waiver out if your insurance information changes.
